Recreating the pillow form is fairly easy. I use a 1/2-inch seam allowance for home dec items. So, just add an inch to the dimensions. As for adding a zipper, you can follow instructions here.
The embroidery is all up to you. This is where you get to be creative. If you zoom in to the picture at Pottery Barn, you can see that the picture that the embroidery actually looks like it is hand done.
